MVA Burrillville, RI Rte-102 (Broncos Hwy) & Central Street November 20, 2009 : Burrillville, RI – Fire Dispatch toned out Stations 1 and 3 for a reported motor vehicle accident in the area of Rte-102 and Central Street around 2350hrs on November 20, 2009. Engine 13, Medic 1, Rescue 3 all called enroute within minutes and dispatch replied with, “We are now receiving several 911 calls regarding this crash”. Engine 13 radioed that they had a two vehicle head on accident with entrapment as they arrived on scene. Station 2 was requested for a second set of jaws due to the drivers in both vehicles being heavily pinned. Firefighter/EMTs climbed into the twisted vehicles to assess the patients and hold cervical spine stabilization. Firefighters readied their hydraulic tools and gained purchase points to being the task of freeing the drivers. They had to use cutters, spreaders, and battery operated reciprocating and circular saws to remove the warped metal.  A hose line was stretched from Engine 21 to protect the scene if a fire was sparked. EMTs initiated IV access inside the vehicles while the extrication continued. The OIC requested a third set of jaws from Station 4 and also for a Lifeflight helicopter to the scene. The driver of the silver sedan was removed in 35 minutes and the black sedan’s driver was removed in 45 minutes. The flight crew was transported to the scene from the landing zone to triage their patient.The silver sedan’s driver was transported ALS to Rhode Island Trauma Center by Medic 1. Rescue 3 brought the flight crew and patient to the landing zone at the middle school. The patient was flown to the trauma center of the University of Massachusetts.

ACW Woonsocket, RI 46 Newton St November 20, 2009 : Woonsocket, RI - Squad 4, Engine 2, Engine 3, Rescue 1, Ladder 2 and Chief 1 responded to the area of 46 Newton Street for a reported structure fire. Squad 4 arrived to find fire in a 2 bay detached garage with 2 vehilces stored inside. Attack lines were placed into operation on heavy fire on side 3 that had extended into the attic space of the garage. Two lines were stretched and operating on the fire. After a quick knock down, overhaul operations began on the underside of the roof rafters and the side 3 outside and inside wall.

Working Fire North Providence, RI 46 Brookside Ave November 18, 2009 : North Providence, RI - The GPS didn't account for heavy traffic on this 15 minute drive to this 2sty raised ranch dwelling fire on Brookside Ave on November 18, 2009. Companies arrived to find this dwelling charged with heavy smoke on both floors. The firefighters found the fire in lower level and quickly contained it with the use of 3 hand lines. The members from Ladder 1 opened the roof the release the trapped smoke in the attic space. The aggressive attack kept the fire conatined to the lower level with heavy smoke damage sustained throughout the rest of the structure.

ACW Woonsocket, RI 173 Newland Ave October 25, 2009 : Engines 2, 3, Squad 4, Ladder 2, Rescue 1 and Chief 1 responded 173 Newland Avenue for a reported basement fire. Engine 2 arrived first due and reported smoke showing from a 2.5sty wood frame occupied dwelling. The firefighters stretched a 1.75 hand line down the side 4 walkway to the basement doorway. They found fire in the C-D corner and went to work with their line. A second 1.75" hand line was stretched to the dwelling but was never charged. The driver of Engine 2 hand dragged his own feeders to the hydrant opposite the truck, but never charged the lines. Tower 2 set up it's bucket to the roof in case of the need to provide vertical ventilation. The OIC request the balance of the working fire assignment bringing Ladder 1 and Rescue 2 to the scene for EMS standby and F.A.S.T.  The fire was quickly knocked down using tank water only. The truckies opened up the exterior siding and also set up their PPV to evacuate smoke from the dwelling. The fire was placed under control in 45 minutes.

ACW Burrillville, RI 698 Douglas Pike October 24, 2009 : Burrillville, RI – 911 calls rang into dispatch reporting a possible explosion in a building on Douglas Pike in the area of the Western Hotel & Pizza Shop around 1610hrs on October 24, 2009. All Burrillville Departments were toned out to respond to the address of 698 Douglas Pike to investigate the reports. Nasonville’s Engine 31 arrived first due and reported smoke showing from a 2.5sty wood frame occupied multiple dwelling. A resident from the first floor was outside of the dwelling with flash burns and smoke inhalation.  A 1 ¾ hand line was stretched into the apartment to provide a quick knockdown of the flash fire. Companies set up a PPV fan on side one to blow any residual smoke out of the dwelling. The State Fire Marshal’s Office was requested to the scene to help identify the cause of the flash fire.
The victim was transported to Rhode Island Trauma Center for treatment of the burns and smoke inhalation.

5th Alarm Glocester, RI 36 First Road October23, 2009 : Glocester, RI – A 5 alarm fire ripped through an occupied dwelling at 36 First Road around 1230hrs on October 23, 2009. All Glocester fire companies responded on the initial alarm. The first arriving officer reported smoke showing from a 20x40 1 story ranch style dwelling. As the first hand line was stretched into the dwelling, a rapidly spreading fire was found on the side 2-3 corner. An aggressive attack was underway as the fire continued to intensify. Command pulled all firefighters out for defensive attack as the fire was now spreading throughout the rear and attic area. Command requested an equivalent of 5 alarms to the scene for tankers, water supply, and manpower. The bulk of the heavy fire was knocked down in an hour. Companies were allowed to reenter the structure to start the overhauling process. The house was destroyed by the blaze and the possible cause and origin was believed to have been clothes dryer in the side 2-3 corner. All of the occupants were safely evacuated from the dwelling.

Working Fire Millbury, MA 26 Shore Terrace October 16, 2009 : Millbury, MA firefighters doused this dwelling fire in roughly 30 minutes at 26 Shore Terrace. The fire heavily damaged the one story occupied dwelling that sat on lake front property. Long large diameter hose lays were required due to the lack of hydrants on this narrow street.

Reported dwelling fire North Providence, RI 25 Brookside Dr October 15, 2009 : I was sitting in the North Providence Fire Alarm Office when the call was taken for a reported dwelling fire. The reported address was a few streets down from the station. Police arrived with smoke showing from a 1sty dwelling. Engine 1 arrived and stretched a dry line as a precaution due to the smoke condition on arrival. The smoke was caused by a delayed ignition of an oil burner.

3rd Alarm Pawtucket, RI 121 Magill Street October 03, 2009 : Pawtucket, RI – Heavy smoke and fire conditions greeted the Engine 1 as they arrived on scene of this occupied 30x60 3sty wood framed dwelling at 121 Magill Street around 0300hrs in the morning on October 3, 2009. The second floor side 3-4 corner had heavy overlapping fire running the stairwell upwards as companies started an aggressive interior attack. Engines 5 and 2 tagged water supplies and also stretched in back lines to try to get a hold on the rapidly spreading blaze. The Battalion 4 requested a second alarm soon after his arrival. The fire was now spreading into the third floor and cockloft of this flat roofed balloon framed New England triple decker. The chief had fire alarm transmit the evacuation tones and to strike the third alarm bringing the rest of the city’s fire apparatus to the scene. Once all firefighters were outside, a manpower roll call was conducted. Ladder 2 and Tower Ladder 2 had a difficult time getting their sticks into position due to overhead wires as several big lines were stretched. Portable guns along with regular 2 ½” nozzles were in place on all four sides as Tower Ladder 1 and Ladder 2 went to work with their pipes. The bulk of the heavy fire was knocked down in an hour allowing the firefighters to reenter the dwelling to get to pockets of hidden fire. The fire was placed under control around 0430hrs. Mutual Aid from Providence, North Providence, and East Providence covered the empty firehouses during the fire. The fire is under investigation by the Pawtucket Fire Department. As companies were mopping up at the 3rd Alarm, the covering companies handled a car fire that extended to a dwelling on Baldwin Street. This was the third fire of the shift for the Pawtucket Fire Department. The first fire was a commercial dumpster that had extended to a vacant 2.5sty dwelling on Centre Street around 1900hrs.

Working Fire North Smithfield, RI 375-B Saint Paul Street September 30, 2009 : North Smithfield, RI – Calls for smoke from the building rang into the North Smithfield Fire Department around 1030hrs on September 30, 2009 for the address of 375-B Saint Paul Street. Fire Alarm stilled out Engine 1, Engine 2, Cars 1, 2, & 3, along with a task force from the City of Woonsocket Fire Department to respond to the scene. Engine 1, whose station is less than a mile from the scene, was out of town training with the Woonsocket Fire Department at a vacant mill complex. Engine 2 was covering the town from Headquarters. Engine 2 arrived first due and reported a heavy smoke condition from an occupied 1 story wood frame dwelling. This dwelling had several irregular shaped additions attached throughout the years.  Engine 2 stretched in a 1 ¾” attack line through a side 2 doorway into the heavily charged dwelling. Lieutenant D. Mowry reported a rapidly advancing attic fire to command. Woonsocket Ladder 1 made the roof via side 2 and opened it up on side 2 over one of the many roof line changes. The truckies encountered a heavy smoke condition in the attic with fire soon making its way through the roof. NSFD Engine 1, Woonsocket Engine 3, Blackstone Engine 51, Millville Engine 2, all arrived on scene to either stretch in back up lines or to help with the extensive opening to expose the pockets of hidden fire. North Smithfield Tanker 1 arrived to feed Engine 2 due to the lack of hydrants in the area. The bulk of the heavy fire was knocked down in 30 minutes. Burrillville Rescue 3 responded to the scene to treat a member of Ladder 1 who sustained a minor hand injury opening up the roof. Mutual aid responded from Burrillville and Smithfield to cover the 2 empty firehouses. This is the second fire in this dwelling in the past 10 years. The cause of the fire is under investigation by the North Smithfield Fire Marshal.

Millville, MA 2 vehicle accident with entrapment Central St & Providence St September 28, 2009 : Millville, MA – Fire Dispatch was alerted to a motor vehicle accident involving two vehicles with one possibly into a utility pole at the intersection of Central and Providence Streets around 2245hrs on September 28, 2009. The 911 phones were ringing in the background as the dispatcher was giving companies the location over the airwaves. A heavy rain storm also was passing through the area during the time of the accident.  A Millville police officer arrived with Fire Car-1 and found a four door sedan in the middle of the roadway along with a SUV into a pole. Both vehicles sustained heavy front end damage. The driver of the sedan was ambulatory on scene but the driver of the SUV was still inside the vehicle. The SUV had side impact damage to the driver’s side door pinning the occupant who was complaining of head injuries. The impact from the SUV into the pole had snapped the pole about 2’ off the ground. Millville Rescue 1 personnel climbed into the SUV to maintain C-spine stabilization and to apply a cervical spine collar before members from Ladder 1 started to pop the door with their Jaws of Life. Engine 2 arrived and command requested that they stretch a 1 ¾” hand line to protect the members while extrication was taking place. Engine 1 also was on scene for manpower to assist with removal of the patient of the vehicle. Firefighters used their spreaders to pry the driver’s door open in about 5 minutes. The patient was removed to a long back board and transported to Rescue 1 for further evaluation. The driver of the sedan was refusing medical aid on scene. National grid was called to the scene to replace the severely damaged pole. The victim was transported to Milford Hospital for further evaluation of her injuries.

Working Fire Providence, RI 58 Hazard Ave September 14, 2009 : Providence, RI – Fire Alarm stilled out companies to the address of 58 Hazard Ave for a reported dwelling fire around 1315hrs on September 14, 2009. Engine 5 arrived to find this 30x50 2 story brick/wood dwelling heavily charged with smoke. Companies found the seat of the fire in the basement and began to stretch lines to extinguish the blaze. The fire was starting to extend upwards but was quickly doused by the aggressive attack. The OIC special called an extra engine and truck over the working fire. No injuries were reported and the cause of the fire is under investigation. The incident commander started to release companies 30 minutes after their arrival.

North Smithfield, RI Rollover with Entrapment Rte146 SB September 12, 2009 : North Smithfield, RI - A single car with 4 occupants was traveling southbound on Route 146 in the area of "West Acres" around 1430hrs on September 12, 2009. The vehicle somehow rolled over down an embankment coming to rest in a patch of trees. A witness reported that 1 occupant was ejected during the roll over, 1 was able to remove himself from the wreckage, and 2 occupants were still entrapped in the wreckage. North Smithfield Lieutenant Dave Mowry was on the first arriving apparatus and requested two more additional EMS rescues to the scene. Engine 1 arrived next due and went to work with their tools to open up the car to free the last occupants. Woonsocket Rescue 2, Lincoln Rescue 2, and North Smithfield Rescue 2 transported all of the occpants to level 1 trauma centers at Rhode Island Hospital and Hasbro Hospital.

3rd Alarm Dudley, MA 40 Fish Rd September 6, 2009 : Dudley, MA - Companies 2 and 3 were toned out around 0145hrs on September 6, 2009 for a reported fire at 40 Fish Road. The reporting party stated that he fled his home that was on fire and ran to a neighbors home to call 911. First due companies requested a second alarm to be struck on arrival due to the heavy overlapping fire from side 1 of the 1.5sty wood framed dwelling. The OIC soon after struck a third alarm bringing Company 1, Southbridge, Webster, Charlton, and nearby Quinebaug Fire from Connecticut. The firefighters stretched in several lines to try to darken down the blaze. The dwelling was located at the end of a narrow road which was a dead end. A tanker shuttle was established but was hampered by this narrow road. Tankers were shuttled in one by one as they were needed to refill the drop tanks. Firefighters were allowed to enter the dwelling to overhaul this severely damaged dwelling about an hour after they arrived. State Police Investigators will be assisting local officials in cause and origin.

Working Fire Mansfield, MA 10 Downs Lane August 21, 2009 : Mansfield, MA - A thunderstorm rolled through the Bristol County region around 1600hrs on August 21, 2009. A bolt of lightning struck a 2 story occupied 40x60 wood framed dwelling at 10 Downs Lane on the ridge line of the main roof causing a small attic fire. Mansfield, Easton, and North Attleboro responded to the scene. The fire was quickly brought under control using 4 engines, a ladder, and an EMS unit.

ACW Burrillville, RI 505 Douglas Pike (RTE-7) August 17, 2009 : Burrillville, RI- All town departments were dispatched to the area of 505 Douglas Pike for a motor vehicle accident involving an utility pole with wires down possible landing on an occupied dwelling. Station 3's chief arrived first due and found a primary wire from the pole to dwelling down on the roadway and pulled off of the dwelling. The OIC had smoke showing from the dwelling on the side 1-2 corner where the electrical meter was mounted to the dwelling. The chief requested National Grid or nearby Pascoag Electric with a push due to the wires still being live. Firefighters stretched  dry line and also dumped a dry chemical extinguisher on the side wall the the meter was on. The dwelling service wires were housed in PVC conduit that ran up and over the side 2 gable end. The conduit began to catch fire from the arcing wires. Dry chemical agent was also dumped on this fire to keep it from extending to the dwelling. Pascoag Electric was closer to the scene and responded in to cut the power at the pole to the downed wire. Once the power was deemed terminated, the firefighters were able to gain entry into the dwelling to check for extension with thermal cameras. They mopped up the PVC conduit fire with an 1.75" hand line. The witnesses on scene reported to the chief that a tractor trailer box struck the low hanging wire as it drove by. The primary wire pulled the conduit off of the dwelling and also ripped the wires out of the meter box.

4th Alarm Douglas, MA 142 West St August 8, 2009 : Douglas, MA – Frantic calls reporting a garage fire next to a dwelling were received at the 911 call center for the address of 142 West Street around 0130hrs on August 8, 2009. Douglas Fire dispatched all fire personnel moments later. As the companies were responding they received word that oxy-acetylene and propane tanks were also inside the structure. Firefighters were encountered with a fully involved 2 bay garage and a 15’ tow behind camper. The camper had two 20# propane cylinders on the tongue of the frame. The occupied 1.5sty wood framed Cape Cod style dwelling had fire running up the side 3-4 corner from the basement level to the roof line.  A 2.5” hand line was stretched in to start knocking the fire down.  A load roar was heard as the 2 propane tanks on the camper started to vent off. The firefighters retreated to a safe distance until all of the fuel had burned itself off. The smoke started to get real thick from the dwelling during this momentary retreat to safety. The OIC special called Uxbridge, Leicester, and Sutton for tankers, firefighters and coverage on his arrival to the scene. Several more hand lines were placed into operation to protect the dwelling from further fire spread. Douglas Ladder 1 set up its stick to start vertical ventilation on side one. Heavy fire started to blow out of the vent hole as soon as the roof was popped open.  The aggressive interior crews waited inside for the heat to vent out of the roof before the pushed in on the second floor and attic. Webster Rescue, Oakland-Mapleville, Pascoag, and Oxford EMS, were also special called to the scene during the fire fight.  All four occupants safely exited the structure before fire apparatus arrived. The firefighters also conducted tedious salvage operations on the first floor of the dwelling to protect valuables from further damage.

ACW Woonsocket, RI Gaulin Ave & Elm St August 3, 2009 : Woonsocket, RI - A sofa and recylner mysteriously caught fire along side this vacant multi story dwelling at the corners of Elm and Gaulin Streets. Quick work knocked this fire down and kept the fire out of the rest of the structure.

Working Fire Uxbridge, MA 33 Julia Dr August 1, 2009 : Uxbridge, MA- A smoke detector alerted this family to get to safety as they were waking up around 0830hrs on August 1, 2009. The homeowner called 911 and reported a possible vehicle fire in the garage of their 2 story wood framed colonial style dwelling at 33 Julian Drive. Engine 1 arrived first and reported “nothing showing” from the dwelling. The crew did stretch a dry line as they investigated the dwelling. They encountered light smoke in the dwelling with an odor of something burning. Companies found the interior of a Honda Pilot involved in fire inside the garage. The crew asked for the line to be charged and quickly knocked the fire down. Engine 2 arrived and stretched a backup line to the dwelling. Tanker 2, Ladder 1, Rescue 1, Engine 3 also responded to the scene when command requested a working fire response. Firefighters placed the vehicle in neutral and pushed it outside after the fire was knocked down. Ladder 1 set up their vent fan to clear the dwelling of smoke.  There was no fire extension to the dwelling.

Working Fire Johnston, RI 2446 Plainfield Pike August 1, 2009 : Johnston, RI - Reports of a well involved occupied dwelling rang in around 0200hrs on August 1, 2009 for the area of Plainfield Pike and Hawkins Lane. Dispatch toned out Engines 1, 2, 3, Ladder 1, Rescue 1, and Battalion 1 for a confirmed working fire.  Fire alarm then radioed to responding companies that people were jumping from the building. First due Engine 1 arrived to find a 1.5 story occupied Cape Cod style dwelling 50% involved. Heavy overlapping fire was venting from the first floor and a 1sty addition on side 3. Command requested Engine 4 along with a 2nd EMS Rescue to be dispatched to the scene. Two occupants had to jump from the second floor windows roughly 12’ to the ground to escape the heavy smoke and fire that was rapidly devouring the first floor living level. Johnston EMS transported one female jumper to a local hospital. The fire was brought under control in an hour using 4-1¾” hand lines. Companies stayed on scene for fire watch and investigation by state and local fire marshals.

Working Fire Plus Providence, RI 186-88 Gallup St  July 20, 2009 : Providence, RI, - The south end firefighters were awoken just before shift change around 0630hrs on July 20, 2009 for a reported fire in the area of Broad Street and Gallup Street. Engine 10 and Ladder 5 arrived first due to find heavy fire conditions on the third floor and attic of a vacant 3sty wood framed 30x60 dwelling. The incident commander requested several other companies over the working fire assignment to the scene. The heavy fire eventually spread throughout the entire attic requiring the OIC to pull all firefighters out for defensive operations. Waterways from Tower Ladders 1, 2, and Ladder 5 played their master streams into the fully involved attic area knocking the fire fire down in 15 minutes. Firefighters were allowed to re-enter the structure to continue fire suppression and overhaul operations. The fire is being investigated by the Providence Fire Arson Squad.

ACW Blackstone, MA 119 King St July 14, 2009 : Blackstone, MA Firefighters responded to 119 King Street for a reported outside fire near a vacant dwelling. The on duty crew responded with Engine 54 while the call stations responded in also. The police cruise assisgned to this grid reported a rubbish fire that was indeed extending to the "D" side sidewall of a 1sty wdfm vinyl sided dwelling. The fire was starting to burn into the siding as the first due units arrived. The firefighters quickly stretched an 1.75" hand line and dosed the rubbish pile. There wasn't any further extension into the structure. The building was reported to be foreclosed on recently. The chief held the scene with two engine companies.

Working Fire Woonsocket, RI 23 Bell St July 14, 2009 : Woonsocket, RI - Engine 1, 2, 3, Ladder 1, Rescue 1, and Chief 1 responded to the area of Bell St and Park Ave for a reported dwelling fire on July 14, 2009 around 0030hrs. Engine 1 arrived first due and found heavy fire extending from the basement to the third floor on side 3 of this 50x70 2.5sty wood frame vacant dwelling. The rapid deployment of the first attack line knocked the bulk of the heavy fire down in minutes in the rear of the dwelling. The fire did however extend inside to all floors and attic requiring interior companies to open up the walls and ceilings to stop the fire spread. Ladder 1 opened the roof on side 4 releasing heavy smoke and then fire to take the heat off of the inside crews. A moderate to heavy smoke condition vented from the structure for minutes questioning the extension inside the building's voids. The smoke lessened up after a quick bottle change and regroup of the Jakes. A quick attack, aggressive overhaul, and wet down kept the roof on this dwelling. This was a great stop by the brother and sister firefighter of the City of Woonsocket Fire Department.

Working Fire Providence, RI 26 Wilson St July 14, 2009 : Providence, RI - This was the second working fire in the city in 20 minutes time. As the first fire was getting mopped up on Veazie Street at O'Neil St, Providence Fire Alarm stilled out companies to the area of Wilson Street and Elmwood Ave for a reported dwelling fire. As the companies responded in, the dispatcher radioed to use caution due to a heavy police presence in the area. Reports were being taken that a woman was in the area possibly carrying large amounts of gasoline and maybe weapons. The police had the suspect in custody as Engine 3 arrived reporting a "Code Red". Fill in companies were also then deployed to the scene. Engine 3's pump lost it's prime during initial attack requiring Engine 8 to step into action. An aggressive interior attack comensed on the heavy fire venting from the second floor of this 2.5sty 30x60 wood framed dwelling. Ladder 1 wasn't able to throw it's stick due to the overhead utility lines. Several lines were ran into the dwelling to knock thi sfire down in 15 minutes time. Truckies opened up the structure as the engine guys stood by to wet down the remains of the fire apartment. No injuries were reported and the cause of yet another suspicious fire in the city will be investigated.

All Hands Fire Uxbridge, MA 196 Rivulet St July 13, 2009 : Uxbridge, MA - Calls for a reported dwelling fire rang into the fire alarm office around 1430hrs on July 13, 2009 for the address of 196 Rivulet Street. Engine 1 arrived to find smoke showing from this Cape Cod style dwelling and called for a working fire assignment to be dispatched. All of Uxbridge stations along with an Engine from the Town of Northbridge responded in to the scene. The crew from Engine 1 lead in with a 1.75" attack line to quickly attack and knock down this kitchen fire. Northbridge Engine 2 streched a back-up line to protect the aggressive attackers on the first line. Uxbridge Ladder 1 set up their PPV fan on side 1 to help evacuate the smoke charged dwelling. Three lines were eventually stretched in while only two were charged and a 5" LDH feeder was laid up to the scene by Engine 2. No injuries were reported.

4th Alarm Boston, MA Box 7281 400 Dorcester Ave area of Old Colony and D Streets July 11, 2009 : 50 minute drive from home after a few minutes of listening, well procrastinating....this is all that was left.........enjoy


About 120 Boston Firefighters worked this 4 alarm fire at the Atlantic Seas Pride warehouse located at the corners of Old Colony Ave and D Street around 0130hrs on Saturday, July 11, 2009. First arriving companies had heavy fire in the 300x100 metal clad 2 story structure located in the South Boston section of Andrews Square. The OIC requested several alarms in concession due to the rapidly deteriorating conditions. Two tower ladders, two ladder pipes, and numerous deck guns along with big lines played water into the complex for 2.5 hours. The building is estimated to be a total loss with a rough damage slated at 2 million dollars. One firefighter did sustain minor injuries during the blaze.  Investigators are still trying to figure out what caused the blaze.

Working Fire Cumberland, RI 1 Pilgrim Terrace June 24, 2009 : Cumberland, RI - This fire at 1 Pilgrim Terrace was the third fire in a weeks time that the Jakes were called out to battle. This fire was in Station 5's district, which is in North Cumberland. Engine 54 and Truck 5 arrived to find heavy smoke and fire venting from the garage of this 1 story ranch occupied dwelling. An aggressive interior attack using two 1.75" hand lines through the breezeway door on side 1 knocked the bulk of the heavy fire down in 5 minutes. The fire was contained to the 2 bay garage which consumed a car and motorcycle. Engine 54 tagged their own hydrant  in front of the dwelling. Truck 1's crew made the roof via their stick to vent the roof if deemed necessary by command. Engine 22 and 41's crew assisted with back up lines, overhaul, and relief to the first due Jakes. Rescue 2 set up EMS rehab on side 1 and provided water bottles until the Special Signal Canteen arrived on scene. Mutual aid from Lincoln and Woonsocket once again filled in the empty Cumberland fire houses. No injuries were reported and the fire is under investigation by State and local agencies.

Working Fire Cumberland, RI 150 Mendon Rd June 23, 2009 : Cumberland, RI - Firefighters responded to the vacant transfer station on the property of the Nunes Disposal at 150 Mendon Road just after 0630hrs on June 23, 2009. The vacant 2 story metal clad structure once was an office and rubbish sorting facility. The property is listed as a "super fund" site. First in companies from Station 1 had fire showing from roof area in the office area. The fire was quickly brought under control using three 1.75 hand lines. Engines 12, 22, 41, Trucks 1, 5, and Rescue 2 along with Chiefs 1, 2, 4, 5 responded to the scene. Mutual aid from Lincoln covered empty firehouses. No injuries were reported during the incident.

Working Fire Cumberland, RI 200 Heroux Blvd Unit 406 June 18, 2009 : Cumberland, RI – Stations 4, 5, 2, and 1 along with Rescue 2 responded to 200 Heroux Boulevard for a reported fire in a condo complex. Engine 41 arrived to find heavy smoke showing from Unit 406. The block of condos consisted of 8 units that are constructed of light weight truss framing. Firefighters lead off by stretching a 1 ¾” hand line through the side 1 front entryway. As companies advanced up to the second floor, a heavy smoke condition was encountered. Engine 41 sat on its own hydrant for water supply. Engines 22 and 54 stretched in back-up lines to the first floor and fire floor. Interior crews found a heavy fire condition in the bathroom and attic area above.  Cumberland Truck 1 assisted with the overhauling on the interior as water was played on the fire. Lincoln Ladder 20 threw their stick to the rook to provide vertical ventilation. Command requested an extra engine from Woonsocket and a ladder from Lincoln to stage on scene. Mutual aid companied covered the empty firehouses and to handle several other calls for assistance throughout the town. The primary and secondary searches proved negative as the apartment was vacant at the time of this fire. The Ri State Fire Marshal’s Office will be assisting in the cause and origin of this fire.

ACW Pawtucket, RI 810 Mineral Spring Ave June 3, 2009 : Pawtucket, RI – Engine 5 was stilled out for a reported grass fire in the area of 810 Mineral Spring Ave around 1200hrs on June 3, 2009. As Engine 5 got closer they radioed that this might be a shed behind the reported address. The company officer asked for the full box to respond as they arrived to find this well involved 2 bay garage with an auto inside. One hand line was stretched and a 5" feeder was laid. The residence was roughly 20’ away from the burning structure but wasn’t threatened. The Pawtucket Fire Prevention Bureau will be investigating the cause and origin. Companies returned in 45 minutes.

2nd Alarm Pawtucket, RI 181 Harrison St June 02, 2009 : Pawtucket, RI – The day shift was performing mayday and rapid intervention training as fire alarm phones were ringing reporting a dwelling fire at 181 Harrison Street around 1130hrs on June 2, 2009. As companies broke away from the drill numerous calls were tended for this address. Engine 2 arrived to find heavy smoke pushing from all floors on side 4 of this partially occupied 3sty balloon framed dwelling. The driver pulled his rig past the structure to leave room for the truck work and next due units. Heavy fire was venting from side 2 on the first floor as Engine 2 was placed into pump. The fire was starting to melt the vinyl siding of 185 Harrison due to the radiant heat. Ladder 2 set up its stick to the roof on side 4 and began to open the roof. Several lines were stretched to the fire floor and above levels to stop this blaze. Primary searches were aggressively performed due to reports of a trapped occupant on the fire floor. Searches proved negative for this occupant but a canine was removed to safety by the Jakes.All of the cities companies were eventually on scene due to the size of the dwelling and fire conditions on arrival.  The first floor of the dwelling was gutted by the rapid fire spread and overhaul. The second and third floors were reported as vacant. The cause of the fire is under investigation by the Fire Prevention Bureau of the Pawtucket Fire Department.

2nd Alarm Northbridge, MA 32 Cottage St June 02, 2009 : Northbridge, MA – Reports of an apartment fire proved correct around 0015hrs on June 2, 2009 when a Northbridge Police officer arrived to find heavy fire showing from a 3sty wood frame occupied multiple dwelling. The newly constructed vinyl sided dwelling housed six apartments. The heavy was venting from the Bravo side from division 1. Engine 1 arrived moments later confirming the working fire. They lead off with a 1 ¾ attack line aggressively making the fire apartment through the Bravo side entrance. This rapid hose line deployment kept the fire contained to the apartment of origin. Primary and secondary searches were performed by the next due companies and confirmed that all occupants were out. As companies were working the fire, command requested a second alarm to be struck bringing the rest of the town’s companies and Uxbridge Ladder 1 to the scene. Northbridge Ladder 1 placed their stick to the roof through a maze of wires and trees. The roof did not need to be opened due to the quick fire knock down. A second line was stretched to the entrance but was not needed. The fire was knocked down in 20 minutes of the first due units. Mutual aid was dispatched to cover the empty firehouses. A resident of the building was treated for smoke inhalation by Upton EMS and transported to a local hospital. The fire is under investigation by the Northbridge Fire/Police departments for cause and origin.

Working Fire Cumberland, RI 35 Hopkins Terrace May 30, 2009 : Cumberland, RI – Several calls reporting a building fire at 35 Hopkins Terrace was received at Cumberland Fire Dispatch around 1930hrs on May 30, 2009. Cumberland stilled out Stations 1, 2, 4, 5 and Rescue 2 as the phones rang in the background. The police cruiser on patrol arrived first reporting heavy smoke showing from a 20x30 1.5sty wood framed occupied dwelling. The firefighters from Station 1 arrived to find heavy fire conditions from side 1 on the second floor. The first line was stretched into action as the fire was rolling overhead. The OIC pulled everyone out due to the rapidly deteriorating conditions. Big lines were placed in operation by the next arriving companies. The fire broke through the roof line on the 1-4 corner and along the side 4 eaves. The heavy smoke column could be seen for miles. The fire was knocked down in 40 minutes allowing companies to return inside for overhaul. Mutual aid from Lincoln Stations 2 and 4 and Woonsocket Engine 4 covered the empty firehouses. One firefighter did receive a minor injury. He was treated on scene for minor burns. The occupants were home at the time of the fire. The homeowner’s child reported that his air conditioner was on fire as the family fled the home to safety.  The child tried douse the flames with a garden hose as his mom called 911. The official cause is under investigation by the town of Cumberland Fire and Police and Rhode Island Fire Marshal’s Office.

I was working my second night in the firehouse. I was out on a medical run clearing the hospital as this fire broke out. My partner and I "happened" by this fire by taking a few wrong turns. The smoke was pushing black acrid smoke which was visible for miles. We drove the rescue into the nearby nursing home driveway, saw the heavy fire and a light bulb turned on in my mind.....use the camera phone or quickly drive a mile to my firehouse to get my camera...we quickly left the scene to retreive my gear. This is what I was able to take in before my portable radio keyed up saying, " Fire alarm to Rescue 4, are you availiable for a run?" ughhh freaking work!!!!!!

2nd Alarm Bellingham, MA 7 Stone St May 21, 2009 : Bellingham, MA – Heavy fire was venting from all floors on arrival of Engine 1 at 7 Stone Street around 3 o’clock in the morning of May 21, 2009. The hydrant on the street was located near the fire building but was inaccessible due to live power lines that fell to ground near it. Engine 1 had to find a water supply on Hartford Ave to establish water supply which was about 300’ from the fire building. Firefighters from Engine 1 stretched two 2 ½” attack lines to commence fire attack. Bellingham Chief 1 ordered a quick second alarm bringing Franklin and Medway to the scene. Franklin Engine laid in to the scene to supply Bellingham Ladder 1 at the A-D corner of the dwelling. Medway Engine assisted in operating attack lines. Milford Fire sent Ladder 1 and their mobile air supply truck to the scene. All of the 4 occupants were confirmed out of the dwelling as companies aggressively attacked this raging inferno. The homeowner heard the smoke detectors sounding as they were asleep on the second floor. They were able to escape outside as heavy fire was rapidly taking possession of their home. The building sustained a major collapse 20 minutes into the fire bringing the entire roof and front side of the structure in on itself. Bellingham Ladder 1 hydraulically overhauled the D side second floor sidewall to expose the hidden fire in the center of the structure.  Several hand lines played water and foam into the shell to knock down the remaining fire. The fire is under investigation by the Bellingham Fire and Police Department and the State Fir e Marshal’s Office. There were also no injuries reported to the occupants and fire personal.

Working Fire Smithfield, RI 99 Pleasant View Ave May 18, 2009 : Smithfield, RI – The police received a burglar alarm for 99 Pleasant View Ave around 2340hrs on May 18, 2009. As the cruisers arrived heavy smoke was pushing from this 3 store plaza at 99 Pleasant View Ave (Rte-116). As the police reported a working fire, Smithfield Fire Alarm was also answering several calls of the fire on the 911 lines. Engines 1, 2, 3, Medics 1, 2 and Chief 2 responded to the scene. Chief 2 arrived to find a well involved 50x100 1sty concrete block building with a trussed roof system. The OIC reported that the fire was going to be fought in a defensive mode as apparatus arrived on scene. Heavy fire was coming from the center store which was Bob’s Place Restaurant and was rapidly extending to the stores on the sides. The left side business was 1 Stop Collectibles and the right being Stop and Go Groceries and Deli. A special call for a ladder from Johnston Fire and an engine from Harmony Fire for RIT was called minutes into the blaze. Several big lines and 1 ¾” hand lines were placed into action through the front glass windows into the ceiling and attic area. This aggressive attack had the bulk of the fire knocked down in 20 minutes. There were no injuries reported. The fire was investigated by the Smithfield Fire Marshal’s Office and possible ruled to be electrical in nature. The building is being considered a total loss.

Working Fire Attleborough, MA 88 North Main Street May 18, 2009 : Attleborough, Ma – Police reported a working fire at the James Sullivan District Courthouse as firefighters were being stilled out to 88 North Main Street around 0630am on the morning of May 18, 2009. First due companies had heavy fire venting from side 1 floor 2 on arrival of this 100x100 two story brick structure.. An aggressive interior attack kept the fire contained to the second floor. Ladder 1 threw their stick to the roof to begin vent work. The OIC recalled the oncoming group to staff the stations and to bring extra equipment to the scene. The bulk of the heavy fire was knocked down in 30 minutes with extensive overhauling to the second floor. The fire was kept out the attic but was reported to have dropped down to the first floor.  Salvage covers were ordered to the lower levels to protect the important document s in this court house. The damage estimate was placed around $500,000.00 dollars. No injuries were reported. The fire is under investigation by the Attleborough Fire Department and the Massachusetts Fire Marshal’s Office.

2nd Alarm North Providence, RI 53 Allen Ave May 13, 2009 : North Providence, RI – Engine 1, just clearing an EMS run, reported to fire alarm that they were investigating heavy smoke in the area around 0850hrs on May 13, 2009. They arrived to find this rear of this 1.5sty 20x40 occupied wood frame well involved at 53 Allen Avenue. Firefighters stretched in through the front door with a big line as heavy black smoke flowed over their helmets. The heavy fire was pushing from kitchen area and the three season room on the side 3-4 corner. Command requested a second alarm bringing in fire apparatus from Providence, Johnston, and Smithfield to work and cover. Several lines were stretched to the fire floor, second floor, and to the rear to suppress this blaze. Ladder 1 went up to the roof to vent and check for extension into the attic space finding negative results. The aggressive attack kept the bulk of the heavy fire contained to the rear of the dwelling  and to the enclosed porch. The truckies opened up the majority of the dwelling to check for further fire spread. The fire was placed under control in roughly 30 minutes. The occupants were home at the time of the fire and evacuated safely as firefighters arrived on scene. Several animals were also rescued and treated at the scene by EMS with oxygen therapy. The North Providence Animal Control Officer cared for the animals on scene.

MVA w/ pin and helo Mendon, MA 21 Hopedale Street May 3, 2009 : Mendon, MA – 911 phones lit up around 0730 on Sunday, May 3, 2009 for a reported single vehicle accident vs tree around the area of 21 Hopedale Street. First arriving units reported that a vehicle hit a tree along the edge of the roadway with the driver heavily entrapped in the car. The male driver in his early 20’s was semi conscious and suffered from multi system trauma. Mendon Fire/Rescue along with Hopedale Fire Ladder 1 used two sets of jaws, cutters, and a ram to free the occupant in less than 30 minutes. The OIC requested a helicopter from UMASS Life Flight due to the extended extrication and trauma that the patient received. Life Flight used the Miscoe Hill School on North Ave for the landing zone. The flight crew was brought to the scene by Mendon Engine 1 to help stabilize the patient who was still entrapped in the mangled wreckage. The driver’s side of the BMW 700 series received the most of the damage after striking the tree. The patient was removed from the vehicle and transferred to the Mendon Rescue where the crew assessed the patient further and treated with advanced life support. The patient was then intubated to protect his airway while in flight to UMASS Worcester.  The scene was held for reconstruction and investigation by the Mendon Police Department.

Working Fire Lincoln, RI 47 Old River Rd May 1, 2009 : Lincoln, RI – Fire Alarm started hitting the tones for Station 7, 4, 3, and Rescue 4 around 2330hrs on Friday, May 2, 2009 for a reported heater fire in the bedroom at 47 Old River Road. As the companies were calling enroute, fire alarm keyed the radio reporting that police were on scene stating that the rear of the building had heavy fire venting out of the windows. Chief 7 arrived on scene reporting a working fire in a 1sty wdfm occupied dwelling with fire going through out.  Station 2 was special called to the scene for the F.A.S.T team along with Rescue 2 for EMS/Rehab.  First due Engine 76 arrived and stretched in the first line for fire attack. Engine 31 established a 5” feeder line to Engine 76. Engine 4 stretched in a backup line to the rear to knock the overlapping fire on side 3. As the crew from Engine 4 and Rescue 4 made the rear deck, a 20 pound propane cylinder from the grill let go and was self venting fuel. They cooled the tank down, shut it off, and removed it to safety.  Firefighters then continued to knock down the bulk of the heavy fire from the deck.  The bulk of the heavy fire was knocked down in 15 minutes. The building sustained major fire damage and required extensive overhauling.  Two firefighters fell through the floor in a rear bedroom because of the heavy charring. The Jakes did not receive injuries and continued to open up. Rescue 2 evaluated the occupants on scene for minor burns and smoke inhalation but they refused an ER evaluation. Lincoln Fire and Police along with the RI State Fire Marshal’s Office will be investigating cause and origin.

General Alarm Northbridge, MA 149-159 Border St April 30, 2009 : Northbridge, MA – Firefighters were alerted of a possible building fire at 149-159 Border Street around 1420hrs on April 30, 2009. First due companies arrived to find heavy fire venting from a 75x150 6 unit 2.5sty wood frame occupied multiple dwelling. The OIC then requested a second alarm due to the heavy volume of fire. Firefighters stretched big lines to start trying to darken down the overlapping fire. The fire had extended up to the second floor and was starting to get in the attic area as second alarm companies arrived on scene. Grafton Tower 1 set up on side A, Northbridge Ladder 1 was on the A-D corner, while Uxbridge Ladder 1 took the rear on side C. Several big lines were also placed into action playing water into the windows on all sides. The bulk of the heavy fire was knocked down in an hour but several pockets of fire still existed. A District 7 Structural Task Force was requested to the scene to help with the extensive overhauling. The fire chief reported that there were no injuries to both residents and firefighters. A family dog was rescued early into the fire. High winds also hampered firefighting efforts by helping fan the flames. The building appeared to be a total loss and is under investigation by Northbridge Fire Department, Massachusetts State Fire Marshal’s Office, and the ATF.

Brush Fire North Smithfield, RI 294 St Paul Street April 28, 2009 : North Smithfield, RI - A passerby stopped by NSFD Station 1 to report a rapidly extending brush fire in the rear of 294 St. Paul Street that was threatening two structures. Engine 1, 2, Tanker 1, Car 2 along with engines from Blackstone and Woonsocket responded to the scene. NSFD Engine 1 arrived to find the fire closing in on two dwellings and a large garage. A quick attack from Lt. Laforge and FF Provost knocked the bulk of the fire down near the structures. NSFD Engine 2 took the right flank of the fire and stopped it's spread while Woonsocket and Blackstone took care of the left flank. Companies were on scene for a few hours wetting down and mopping up.

ACW Woonsocket, RI 404 Bernon Street April 27, 2009 : Woonsocket, RI – Fire Alarm stilled out Engines 2, 3, 4, Ladder 1, Rescue 1 and Chief 1 to 404 Bernon Street for a reported fire in an occupied apartment on April 27, 2009 around 1045am. As companies were responding, fire alarm received several more calls reporting the fire. Engine 2 arrived and reported nothing showing from a 2sty wood framed apartment structure. Engine 2’s crew began their investigation on the first floor and found a smoke condition. The radio then opened up with, “Charge my line”! An aggressive interior attack kept this fire from spreading further. Ladder 1’s stick was thrown to the roof through a maze of wires incase of the need for ventilation. The fire was knocked down using tank water and 1 ¾” hand line. There were no reported injuries and the cause is under investigation.

2nd Alarm Attleboro, MA 42 Frank Mossberg Dr April 26, 2009 : Attleboro, MA - Fire alarm received the master box for General Metals Finishing at 42 Frank Mossberg Drive around 2200hrs on April 26, 2009. The building housed a metal finishing company that specializes in gold electroplating and had considerable chemicals stored inside. The first due arriving companies on this box reported heavy smoke showing from the rear of this 60x200 1 story concrete block commercial structure. Firefighters found heavy fire in a rear room that had extended through the roof. Within minutes of the initial response, firefighters reported that a section of the roof had collapsed in the rear of the main building.  Attleboro Ladders 1 & 2 prepared their waterways to flow water down on the raging blaze. The OIC requested a second alarm bringing in the rest of the on duty personnel along with mutual aid from North Attleboro and Pawtucket, RI. Pawtucket Engine 4 laid in a supply line and fed the fire department connection to charge the building's sprinkler system on side b. Firefighters were allowed to enter the structure once the utility company confirmed that the power was terminated. The fire was declared under control about 2330hrs. Fire companies remained on scene for quite some time mopping up and picking up the 1000 plus feet of large diameter feeder lines. The Attleboro Fire Department and State Fire Marshal's Office will be determining the cause and origin of this fire.

Working Fire Worcester, MA Box 4-19 59 King Street March 30, 2009 : Worcester Fire Alarm dispatched companies to Box 04-19 for 59 King Street just after 1230 in the afternoon on a wet Monday on March 30, 2009. Engine 2 arrived to find smoke showing from the A-D corner of a vacant 40x60 2sty wdfm dwelling. Firefighters stretched a 1.75" handline through the front door to start the fire attack. A second 1.75" hand line was dragged in for back-up. Companies found fire on the second floor A-D corner in a closet that had extended slightly to the main room. Ladder 3 opened the windows on side 1 floor #2 and then made the roof to open if needed. The fire investigation unit was requested to the scene due to the nature of this fire. The scene was placed under control in roughly 30 minutes.

3rd Alarm Pawtucket, RI 73 Baldwin St March 13, 2009 : Pawtucket, RI - Engine 5 arrived first due finding heavy smoke showing from this 3sty wdfm flat top vacant dwelling at 73 Baldwin Street on Friday the 13th around 2015hrs. Companies found heavy fire conditions in the basement rapidly extending upwards through the front stairwell. The heavy smoke and fire blew out of the second floor windows on side 4 and 1 once companies beagn to vent. The OIC struck second and third alarms due to the heavy fire that was pushing from the dwelling. All companies were ordered out of the building for roll call and for a quick defensive operation. Engine 5's deck gun and Tower Ladder 2's waterway knocked the bulk of the fire down and companies were allowed to return inside to mop up. Pockets of hidden fire were soon exposed and wet down. The fire was placed under control around 2130hrs.

2nd Alarm Burrillville, RI 323 Mt. Pleasant Rd March 11, 2009 : Burrillville, RI - Firefighters in the town of Burrillville were once again called upon to fight their second dwelling fire in 24 hours. The second fire was located at 323 Mt. Pleasant Rd in the Nasonville section of the town around 0700hrs. Engine 31 arrived to find heavy fire conditions in a garage and breezeway attached to a 1.5sty wood framed occupied dwelling. Firefighters stretched a 2.5" hand line to start the attack of the heavy overlapping fire on side 1. Oakland-Mapleville Engine 11 arrived second due and backed up Engine 31 with a 1.75" line through the front door to keep the fire out of the dwelling. 31's crew then worked the big line from side 1 to sides 4 and 3 to continue knocking the bulk of the fire down. The OIC had companies begin interior overhaul once the fire conditions were darkened down. The heavy fire was contained to the breezeway and garage with some fire extension, heavy smoke, and water damage to the living area of the dwelling. Mutual aid was received from North Smithfield, Blackstone, Chepachet. No injuries were reported and the cause of the fire is under investigation.

2nd Alarm Burrillville, RI Brook Rd & Round Top Rd March 10, 2009 : Burrillville, RI - Fire Alarm received calls for a fire at the RI DEM maintenance garage on the corner of Round Top Rd and Brook Rd around 0630 this morning. Firefighters from the Harrisville District were first due and requested a second alarm to be struck due to heavy fire conditions and also lack of water supply. A tanker task force was requested to the scene on this second alarm. Several big lines and a deck gun were used to knock the bulk of the fire down. Two Oakland-Mapleville engines set up on Brook Rd just down from the fire to supply the scene from a draft site while another engine set up a tanker fill site from further down stream. The building was constructed with heavy timber in a "U" shaped design. The structure was like a car port with a shed roof over it all. There were not any doors to the garage. Two dump trucks, a tractor used for roadside mowing, a medium size back hoe, and several pick up trucks were inside of this storage area at the time of the fire and were all completely destroyed.

3rd Alarm Providence, RI Kossuth Street & Hyat Street March 08, 2009 : Providence, RI - Fire Alarm received calls for a vacant dwelling fire in the area of Kossuth and Hyat Streets in the Manton section of the city around 2100hrs on March 08, 2009. Engine 15 arrived first due finding heavy fire in a vacant 3sty 40x60 wood framed dwelling extending to a similar structure on side 2. Heavy fire was pushing from all floors up to the attic in this balloon frame mansard roofed structure. Engine 15 requested a quick second alarm to be struck due to the overlapping fire. Several "big" lines were stretched to protect the exposures and to start knocking the fire down. Tower Ladders 1 and 2 set up their waterways on Kossuth Street side to blitz the original fire building. The siding on the exposure suffered heavy damage with some minor extension inside the dwelling. Firefighters were allowed to re-enter the dwelling to chase hot spots and open up once the bulk of the fire was knocked down. Three injuries to firefighters were reported and the fire deemed suspicious and is under investigation.

Springfield, MA Buff Trip March 07, 2009 : Springfield, MA - Fellow fire photographers Scott LaPrade, Alan Brackett, Dave Mowry, Dave Cinq-Mars, Eric Moreau and myself traveled to Springfield, MA for the hope of shooting some incidents. We arrived in the city just after 0900hrs and returned around midnight. We started off with a working fire response to 3601 Main Street for a reported commercial structure fire. Engine 10 arrived to find the rear of a Baystate Medical Center bus on fire. Workers had the bulk of the fire knocked down prior to the first companies arrival. Engine 10 stretched a 1.75 line to wet down the involved area. Tower Ladder 1 also remained on scene to assist. We then responded to the area of 109 Johnson Street for another reported dwelling fire around 1400hrs. Engine 3 arrived first due and reported smoke showing from the chimney of an occupied 3sty wood frame dwelling. Firefighters made their way into the dwelling to investigate to basement and found heavy fire conditions. Heavy smoke started to issue from the front door as next due companies arrived. The truckies began the task of opening up the windows on all floors. Smoke was pushing from all floors, eaves, and roof vents as water was being placed on the fire. Three hand lines were stretched and operated to knock the fire down and vent the upper floors. One firefighter received a minor hand injury and was treated by AMR personnel on scene. The fire was knocked down with in 30 minutes and companies remained on scene for 1.5 hours. One hard arse police lieutenant tried to push us away from the scene as seen in photos 026 and 027. This police officer was very rude and obnoxious to us....great job sticking up for yours and our rights Scott!

W/F Cumberland, RI 806 High Street March 02, 2009 : Cumberland, RI - Stations 1, 2, 4, 5 and Rescue 2 were stilled out to 806 High Street in the area of Baldwin Street for a reported dwelling fire around 1600hrs on March 02, 2009. Station 1 units arrived to find heavy smoke showing from a 1.5sty wood frame dwelling that appeared to be occupied. Heavy fire conditions were found on side 3 extending upwards to the soffit. Firefighters pulled a 2.5" big line from Truck 1 through the side yard to start fire attack. Engine 22 arrived second due and secured a LDH feeder into the scene. As companies were knocking the bulk of heavy fire down, the OIC received reports of a possible trapped occupant. Truck 1 then dropped their big line and started an aggressive interior search. Third due Engine 41 stretched a 1.75" attack line off of Truck 1 into the second floor to check for extension. Interior crews did find extension on the second floor and also into the addition off of side 3. Truck 5 set their stick up to the roof on side 2 for vent work.  Lincoln Engine 4 and Ladder 51 responded in on mutual aid. Engine 4 assisted in primary and secondary searches and then overhaul operations. Ladder 51 set up on side 1 as the F.A.S.T. company. Cumberland Rescue 1 and 2 had the EMS rehab sector established. The fire was under control in roughly an hours time. The fire is under investigation by the Cumberland Fire and Police Departments along with the RI Fire Marshal's Office. No injuries were reported and a family dog was safely rescued from the dwelling.

ACW Uxbridge, MA 47 Chestnut Street March 02, 2009 : Uxbridge, MA - Uxbrdige Fire received calls for a dwelling fire at 47 Chestnut Street as mother nature dumped 12" of snow onto the area. Police arrived first due and reported fire on the second floor in the laundry room of this 30x60 2.5sty wood frame occupied dwelling. All occupants were out of the dweeling as the fire department arrived. Ladder 1 and Engine 2 arrived and quickly knocked this dryer fire down with a water can. A 1.75" handline was stretched and charged as a precaution but was never used. Engines 3 and 5 also responded to the scene but were released soon after command gave the under control to dispatch. Engine 2 was just placed into service and has seen her fair share of work in 2 days. This has been her 3rd minor fire since being in service.

2nd alarm Woonsocket, RI 385 Park Ave February 25, 2009 : Woonsocket, RI -  As firefighters were mopping up the working fire on Valley Street, Fire Alarm cracked the airwaves, "Fire Control to Valley Street IC, getting reports of a second building fire on Park Ave near Crawford Street". IC, we're getting reports from the police that this building is occupied and there is fire on the side of the dwelling extending to the roof line. The Chief and Rescue 1 arrived to find a large 3sty wood framed 30x80 occupied apartment house with fire on the side 2 porch extending up to the dormer area and roof line. The chief ordered a working fire which brought in out of city companies along with Woonsocket Engine 4. Police officers were in the building helping occupants outside while Rescue 1's crew started primary searches. The firefighters from Engine 4 arrived from the other side of the city to stretch first hand lines on this rapidly spreading fire. North Smithfield Ladder 2 arrived and set up on the 1-2 corner to start roof ventilation operations. Woonsocket Engine 1 and Tower 2 were freed from Valley Street fire to assist operations. Engine 1 established water supply to Engine 4 and Tower 2. This fire was Tower 2's first fire. The tower was set up on the 4-1 corner to open up the gable ends on side 1 and 4. Lincoln Engine 1, Burrillville Engine 32, Lincoln Rescue 2, and Blackstone, MA Rescue 1 also responded to the scene. Lincoln Engine 1 helped stretch the second back up line to the second floor to attack the fire in the walls and ceiling. Blackstone and Lincoln Rescue crews also were deployed to the upper floors to help open up the balloon framed walls and tin ceilings. The building had been renovated over time creating voids that helped the flames spread. The fire did extend into the attic area and had broken through the roof on side 2. An aggressive interior attack on floors 2, 3, and in the attic kept the fire mostly to side 2. Heavy smoke was pushing from all of the eaves but was reduced to residual smoke due to the work of the firefighters inside. Several occupants were treated for smoke inhalation on scene. No firefighter injuries were reported.

W/F Woonsocket, RI 160 Valley Street February 25, 2009 : Woonsocket, RI - Firefighters responded to the area of Valley and Cross Streets for smoke from a dwelling around 0100hrs on February 25, 2009. Engine 3 arrived to find smoke showing from a 2.5sty wdfm vacant dwelling. As Engine 3's crew stretched in, Ladder 1 set up their stick to the roof through a series of wires and tree branches. Interior companies found fire on the first floor extending upwards through the walls of this house under renovation. The fire traveled upwards into the attic space and broke out of the gable end trim boards. Firefighters brought a second hand line to the front porch roof to knock the fire down as the truckies opened up. The bulk of the fire was knocked down in about 20 minutes. As the firefighters were mopping up, Fire alarm keyed the airwaves reporting another possible building fire in the area of Park Ave and Crawford Streets. Police reported on scene with a large 3 story occupied dwelling with fire on the  second floor porch extending to the roof line.

Working Fire+ Providence, RI 345-47 Willard Ave February 16, 2009 : Providence, RI - Providence Engine 10 called smoke showing from their quarters on Broad Street on February 16, 2009 around 1500hrs for a reported dwelling on fire at 345-47 Willard Ave. The "Broad Street Bullies" arrived to find a 30x50 2.5sty wdfm occupied with heavy smoke and fire rapidly extending from the 2nd floor. An aggressive interior attack was initiated by the 10's and 11's with Ladder 5 going to the roof to open up. The OIC pulled all interior companies out of the dwelling about 15 minutes into the battle due to the deteriorating conditions on the upper floors on side 3 and 4. Heavy fire began to vent out of the eaves and dormers once all companies were outside safely. Deck guns, ladder pipes, and tower ladders doused the heavy overlapping fire down allowing the interior operations to continue again. The upper floors were reported to be under construction at the time of the fire allowing the fire to take possession a lot quicker. The first floor was occupied at the time of the fire and all of the occupants did safely evacuate as companies arrived. Several special called companies supplemented the working fire assignment. One firefighter was transported for an unknown injury early into the fire. The cause and origin is under investigation by the Providence Fire Department.

5th Alarm Nashua, NH 126 Vine Street February 9, 2009 : Nashua, NH Fire Alarm received Box 658 for Lake and Vine Streets around 0930hrs. First due units reported heavy smoke on arrival from a large 3sty wdfm mixed occupancy structure. The "Taxpayer" occupied 3 businesses and 17 apartments. Firefighters attempted several aggressive interior attacks during this battle. Command had companies withdraw several times to knock down the heavy overlapping fire from the flat top roof line. Companies were allowed to re-enter the structure around 1230 to finish opening up and finding hidden pockets of fire.

MVA Burrillville, RI Rte-102 (Broncos Hwy @ Inman Rd. February 2, 2009 : Burrillville, RI (Providence County) - Stations 3, 2, and 1 responded to Rte-102 (Bronco's Highway) at the intersection of Inman Road on February 2, 2009 for a reported 2 car accident with possible entrapment. First due units from Station 3 found two vehicles in the northbound lane with moderate damage. Chief 3 reported that he had several injuries in the two vehicles. The "Jaws of Life" were stretched from Engine-32 as a precaution. The firefighters were able to remove the occupants from their vehicles without the use of hydraulic tools. Four EMS units transported the patients in full cervical spine precautions to local hospitals for observation.

MVA Rollover with Entrapment Lincoln, RI Rte-295 Southbound nr Rest Area January 28, 2009 : Lincoln, RI (Providence County) - Lincoln Fire and Police dispatch received calls for a reported single vehicle roll over on Route-295 Southbound in the area of the rest area around 2320hrs on January 28, 2008. The roads in the area were starting to get icy as temperatures dipped below freezing after a wintery mix passed through the area throughout the day. Lincoln Police arrived to find a single vehicle rollover off of the roadway into the snow packed shoulder. The officer reported that the sole occupant was alert and oriented but trapped in the severely damaged SUV. Rescue 2 under direction of Lt Willerval reported that Squad-21 from Station 2 was required to do a quick door pop to free the driver. Engine 24 arrived to set up a blocking position and also to provide extra hands along with hose line protection to the members working the scene. The firefighters from Station 2 quickly freed the driver from his vehicle using the preconnected spreader from the squad. The driver was removed using full c-spine precautions to the awaiting stretcher on the roadway. The patient was quickly evaluated and transported ALS to Rhode Island Trauma Center by Rescue 2.

Working Fire North Smithfield, RI 800 Central Street January 26, 2009 : North Smithfield, RI (Providence County) - Fire Alarm received 2 calls for smoke coming from the building at 800 Central Street around 1515hrs on January 26, 2009. Off duty NSFD Fire Lieutenant Dave Mowry arrived first due reporting "smoke showing" from a 200x500 2sty metal clad warehouse and office structure. Light to moderate smoke was pushing with evident fire damage on the upper corner the side 2-1 corner. Engine 1 arrived and stretched a 1.75 attack line through a loading dock door up to the second floor office. Millville, MA Engine 2 arrived and established a water supply from their 2500 gallon tanker to NSFD Engine 1. North Smithfield Ladder 2 set up on the 1-2 corner and proceeded with roof ventilation operations. Woonsocket Engine 6 stretched in the back up line to the office area to assist first due firefighters. The fire quickly knocked down in the partitions on the side 2 wall without further extensions. The building was vacant to active business at the time of the fire. The building owner was on scene prior to arrival of the fire department reporting leaking pipes that were once frozen in the walls.

Personel found a lone deer some distance down the river bobbing in the frigid water. Engine 3 launched Marine 1 into the river from the School Street launch area and headed down stream to retrieve the animal. The members on board snared the deer and pulled it safely to the boat. Marine 1 headed back to the launch with a firefighter holding onto his new buddy. A DPW front-end loaded awaited near the ramp to scoop up the lethargic deer to bring it back onto dry land. Firefighters and DEM agents dried the deer off with towels and blankets from the Rescue. The deer attempted several attempts to upright itself without success due the icy ground. DEM waited by the scene to continue care of the animal.

Working Fire Woonsocket, RI Arnold Street January 18,  2009 : Woonsocket, RI Firefighters responded to 85 Arnold Street for a reported building fire on January 18, 2009 during a 12" snow fall. Engine 3 arrived to find heavy smoke showing from a 3sty brick apartment building. Engine 3's truck would not engage into pump as heavy fire started to blow out of the first floor windows. Engine 6 took over first due duties and stretched in to quickly knock the bulk of the heavy fire down. 
Companies overhauled and checked for extension on the upper floors with negative results.

Working Fire Woonsocket, RI 134 Asylum Street January 9, 2009 : Woonsocket, RI (Providence County) – Fire Alarm stilled out Engines 6, 1, 3, Ladder 1, Rescue 1 and Chief 1 to 134 Asylum Street in the Fairmount section of the city for a reported building fire. As companies were turning out, fire alarm opened up the vocals reporting several calls were being taken also with police on scene reporting a working fire. On arrival, Ladder 1 reported heavy smoke showing from a 30x50 2.5styy wdfm dwelling.

Engine 1 arrived and stretched a 1.75” attack line to the side 4 doorway into the dwelling to begin the fire attack. Heavy smoke started to issue from the structure as the door was forced open. The windows on the first floor level of side 1 began to show heavy fire rolling across the ceiling. Engine 1’s crew aggressively pushed into the first floor battling heavy smoke and fire conditions. The first floor then flashed sending heavy fire racing up the entire front side of the dwelling.

Engines 6 and 3 arrived to help with back up lines and feeders. Engine 6 should have been first due but they were out on a medical run at the time of the still. Several lines were stretched into the structure from side 1 and 4. Ladder 1 made the roof and placed a vent hole on side 4 releasing the heat and smoke trapped in the attic. Engine 2, Ladder 2 (FAST), Rescue 2, along with other command/investigative cars responded into the scene to support units on scene

The bulk of the heavy fire was knocked down in roughly 40 minutes. Firefighters continued to open up and overhaul finding pockets of hidden fire. No injuries were reported. A report of up to 4 occupants lived in the dwelling but it is unknown if they were home at the time of the fire. The Woonsocket Fire Department and State Fire Marshal’s Office will be investigating cause and origin.

3rd Alarm North Smithfield, RI 373 Eddie Dowling Highway (Rte-146A) January 5, 2009 : North Smithfield, RI (Providence County)- The North Smithfield, RI firefighters were faced with another multiple alarm in 12 days. This time the North Smithfield, RI Fire Dispatch was notified of a dwelling fire at 373 Eddie Dowling Highway (Rte-146A) around 4:15hrs on January 5, 2009. Engine 1 arrived to find heavy smoke showing from an irregular shaped 1 and 2 story wood frame dwelling. The occupants that were home at the time of the fire escaped safely without injuries. Engine 1 stretched an 1.75" attack line in through the front door and found heavy fire rolling through the center portion of the dwelling. The fire was rapidly progressing as next due companies arrived on scene. The incident commander requested a 2nd alarm tanker task force due to lack of hydrants on that section of roadway. The fire extended from the first floor into the attic area and to the walls of the second floor addition over the garage on side 2. The fire broke through the roof line of the second floor while companies were inside opening up. An aggressive attack by the interior firefighters knocked the spreading fire that was taking charge in the attic space. A third alarm for tankers and coverage had now been struck bringing in mutual aid from Rhode Island and Massachusetts. The evening rush hour was now being hampered by this fire as both sides of Route 146A were now closed forcing motorist to find alternate paths home. Companies remained on scene for hours chasing hot spots and overhauling. The fire was possibly caused by a fireplace chimney but is under full investigation by the North Smithfield Fire Department and State Fire Marshals offices.

Chimney fire with extension Douglas, MA 93 North Street January 1, 2009 : Douglas, MA firefighters responded to 93 North Street on January 1, 2008 around 2355hrs for a reported chimney fire. Companies arrived to find heavy smoke pushing from the chimney of a 1sty wood frame occupied ranch style dwelling. A quick investigation found that the fire in the flue was extending to the walls and attic in the living room wall that adjoins the breezeway and garage. A working fire was requested by command bringing in mutual aid from Northbridge, Sutton, and Uxbridge. Douglas Ladder-1 set up their stick to the roof to cut a vent hole near the chimney. Two lines were stretched into the dwelling to knock the fire down as companies opened up the walls and ceilings, Salvage covers were draped over the occupants belongings to keep damage to a minimum. The fire was placed under control in about 45 minutes. The occupants and a few pets were able to escape without injuries.
